This Science Video Worksheet is one page and has an answer key in a very simple format that is designed for quick correction. It also has links to a shared drive so you can have Google Forms, MS Forms and other formats.  This video is designed to help introduce 5-8th grade students to some astronomy related to the Earth and Moon: the Earth's rotation, shadows on Earth, the Moon's orbit and Moon phases, gravity's role on Earth and keeping the Moon orbiting the Earth.
