Freebie! This template contains all you need for planning a small group guided reading lesson and recording notes on each reader. The planning section includes spaces for objective or teaching point, title, level, before, during, and after reading activities and a section to list questions to ask about the text. The anecdotal section provides a space for each student's name, a place to record a compliment you give him/her about reading, a section for you to record research you do as he/she reads, and a section to record future teaching objectives.
