Special Education IEP Transition Assessment Interest Inventory Spanish and English Versions

 

This is an informal Special Education Transition Assessment for secondary students that receive Special Education Services.

The file contains two versions: English and Spanish.

The Spanish version PDF is 5 pages.

The English version PDF is 4 pages.

 

(NOTE: the English version has been available in my store for many years; however, I have had multiple requests to translate it into Spanish. I have included the English version in this file for two reasons: (1) It will help Special Education teachers that do not read Spanish to know what each question is asking on the Spanish version because the layout is the exact same. (2) I do not read Spanish and I make NO GUARANTEES that the English to Spanish conversion is accurate or says what it should say. The English version is what I translated; however, I do know that words can translate differently in various languages; therefore, I cannot guarantee that the translation reads correctly.)

 

The Transition Assessment is easy to complete because the questions are designed in a “complete the sentence” format and check boxes.

 

Below are some examples of a few of the “complete the sentence” questions that are included in the assessment:

 

“When I have free time, I like to….”

“If I could choose between playing a video game, watching my favorite TV show or using a computer, I would choose…….because…..”

“My family and friends would describe me as …”

“I think I am best at …..because….”

“I learn best when….”

“When I finish school, the job I hope to find is ….”

 

There are (5) sections that are “check all the boxes that apply to you”

The check box sections and some of the choices include:

 

“I think that I am”

A few of the choices are: responsible, artistic, friendly, shy, etc.

 

“I prefer”

A few of the choices are: to work outside, to build things, etc.

 

“I usually”

A few of the choices are: finish a task I start, frustrate easily, etc.

 

“I am a good at”

A few of the choice are: organizing things, using my hands, yard work, etc.

 

“I think I struggle with”

A few of the choices are: pay attention, following directions, math, using time wisely, etc.

 

Although this is considered an informal Special Education transition assessment, I use it for all my students because it provides great information at the beginning of the year as well as for developing IEP profile pages and behavioral goals.
