Designed with upper elementary in mind, but it's not grade-specific. If you have Canva, you can make it suit your classroom.
Most of this is self-explanatory, but a few things to note:
- Since there are strengths and struggle sections, the “general notes” might be more about moods / behaviors, or things more social/emotional, and less about subject matters, though it’s up to you ultimately.
- This isn’t necessarily comprehensive, so you may discuss verbally more nuanced things (e.g. how to correct a behavior, or something that isn’t totally formed or decided yet).
- On the snapshot page, only check relevant items.
- On the scales page, you’ll place a symbol, sticker, or circle on the line to show how much the student fits the scale; it’s to show the parents what you’ve noticed, and to discuss if any of that doesn’t match what the parents see. (scales aren’t necessarily opposites).
- On the Qs/concerns page, give to parents to fill out or write yourself in real time; circle back or discuss either way.
- on self-eval, give to child in advance.
- Feel free to exclude or edit any of these (editable with Canva link).
