FREE End of Year Class Survey Digital Version for Google Forms

This is an End of Year Class Survey. You will need to edit the google form and change to your name and classes. I also recommend going to the responses tab and creating a spreadsheet for the answers. I wanted my survey to be anonymous so I did not ask for their names or email addresses, but are free to change that too.
