FREEBIE End of Year Class Survey

This is an End of Year Class Survey. You will need to edit the form and change to your name and classes. I also recommend going to the responses tab and creating a spreadsheet for the answers. I wanted my survey to be anonymous so I did not ask for their names or email addresses, but are free to change that too. To see some of the questions asked click on the other pictures.
