Book Report Forms - FREE

This is the book report form that I use at the end of all my novel studies, but it is great to use as a stand-alone resource. There are two different forms:

• The first form has a spot for the students to write in the name of an illustrator, and the option to write or draw their favorite part of the book.
• The second form does not have a spot for the students to write an illustrator’s name and has lines to write in their favorite part of the book.

Ways to use this product:

• print it out
• use as an oral discussion guide
• leave up on computer and let students write answers on notebook paper or whiteboard
