Description
I created this little “Get To Know You” form as a fun way to introduce the students to Health Science. I thought it would be fun to theme it in a medical style and ask a few playful questions that weren’t the typical ones they were accustom to.
Two versions are included: a printable PDF and a fillable PDF. Instructions for printing are also included!
