Description
This is a page that I hand back to my students who are eligible for test corrections or need to come in for tutorials for extra work. I will attach it to their latest test with their test grade written on it and the students are required to fill it out and return it back to me with a parent signature. This is very useful for keeping parents involved with their kids' grade in my class without emailing or calling several parents after each test. I have kept this file as a word document so that you may edit this document to fit your specific needs.
